JASA & Jamaica Council of Persons with Disabilities (JCPD) will be having a mass Registration on Saturday, June 16 between 2 pm – 4 pm @ the Kingston YMCA.
It is also important to register your child with JCPD so that they can have numbers of persons requiring services now but more importantly to help them plan for future resources and services.
There are also numerous benefits to be received once registered with JCPD.
Please find attached the necessary forms to be filled out.
- Remember to register, you will need to fill out the attached Registration Form. Your child’s doctor will need to fill out the Medical Report, preferably the doctor who diagnosed your child. You will need to bring these with you. (For those of you who gave Dr. Samms-Vaughan your forms at our last meeting, they will be waiting for you when you register). - JCPD has arranged for the IDs to be done at the same time (while you wait service). The total cost for the ID is $300.00. Remember the ID will entitle your child to all the benefits that JCPD has to offer.
